For those whose PTSD symptoms are severe enough to prevent consistent employment, Social Security Disability Insurance benefits may be available.

How the SSA Evaluates PTSD

The SSA evaluates PTSD under Listing 12.15 — Trauma- and Stressor-Related Disorders — in its Blue Book. To meet this listing, you must demonstrate:

Exposure to actual or threatened death, serious injury, or violence

Subsequent development of specific symptoms, including intrusive memories or flashbacks, avoidance of trauma-related stimuli, negative mood changes, and heightened arousal and reactivity (such as hypervigilance, exaggerated startle response, or sleep disturbance)

AND either extreme limitation in one, or marked limitation in two, of the following areas: understanding and applying information, interacting with others, concentrating and maintaining pace, and adapting and managing oneself

OR a documented serious and persistent PTSD condition lasting at least 2 years with evidence of ongoing treatment and only marginal adjustment

Qualifying Without Meeting the Listing

If your PTSD doesn’t meet the exact criteria of Listing 12.15, you may still qualify through a medical-vocational allowance at steps four and five of the SSA’s evaluation. The SSA will assess your Residual Functional Capacity (RFC) — what you can still do despite your symptoms — and determine whether any jobs exist that accommodate your limitations. PTSD commonly causes significant limitations in social functioning, concentration, and stress tolerance that can eliminate most work environments.

PTSD and Co-Occurring Conditions

PTSD frequently co-occurs with other disabling conditions, including depression, anxiety disorders, traumatic brain injury, chronic pain, and substance use disorders. When evaluating your claim, the SSA must consider the combined effects of all your conditions. Multiple impairments together may meet or equal a listing even when no single condition does.

Critical Evidence for PTSD Claims

Records from a psychiatrist or psychologist documenting your PTSD diagnosis and symptom history

Therapy or counseling records showing ongoing treatment

Documentation of hospitalizations, crisis episodes, or emergency mental health care

Detailed functional assessment from your treating provider

Records from any VA mental health services (for veterans)

Statements from family members about how PTSD affects your daily functioning

Veterans with PTSD: Additional Considerations

Veterans who receive a VA disability rating for PTSD are not automatically entitled to SSDI — the two programs use different eligibility standards. However, VA records are important evidence that the SSA must consider. A 100% VA rating for PTSD can be compelling evidence in an SSDI hearing.

PTSD can be an invisible but profoundly disabling condition. You deserve the support you’ve earned.
